We want to get more specifics from our visitors when they submit a contact form. In addition to the standard fields, we will add a field titled “This is about” and give them a series of checkboxes to select. We are choosing checkboxes instead of a drop-down because it’s a little easier for visitors browsing from a mobile device to navigate.

From the Contact Enhanced dashboard, click on Form Fields from the left sidebar menu.

Click New.

Enter the name of the field.

Select the field type. We will select Checkbox.

Click the Save button from the top toolbar.

In the Value field, we will add the options we want people to be able to check. Ours will look like Social Network|Events|Specific Dog Question|Specific Cat Question|Specific Reptile Question|Other.

Add a Tooltip in the Tooltip tab.

Add any Attributes into the Attribute tab.
